Bulk import error: "Missing mandatory field in header: name"

Issue Symptoms

When I attempt to do a bulk import of users with a CSV file, I receive the error, Missing mandatory field in header: name.

Resolution Steps 

The name field is required when bulk importing new users from a CSV file. Unless you are updating existing users, you must also include the email and restriction fields along with the name field. 

Add a column of the user's full name to your CSV file and include "name" in the header row of the file.
